Supporting Life Goals of Adults with I/DD

Jubilee Awarded Grant from Phillips Charitable Foundation

Thanks to a grant Jubilee received from the Phillips Charitable Foundation, our Person-Centered Planning Team participated in intensive training to further improve our work supporting the goals of the people we support. The training, from the Council on Quality and Leadership (CQL), will help Jubilee incorporate CQL best practices into our Person-Centered Plans.

“We are honored to support the CQL Training for Jubilee Association of Maryland staff, which will in turn help their clients reach their individual goals and provide measurable improvements in their programing,” shared Eileen Gross, Founder and President of Phillips Charitable Foundation.

Person-centered planning helps Jubilee determine what goals and outcomes each person we support would like to work on for the upcoming year, and what success looks like to that person. It acts as a roadmap on what services to provide.

Incorporating CQL will give Jubilee a better framework for this process by exploring more focus areas such as: safety, health, continuity, exercising rights, fairness, living in integrated communities and environments, participating in the community, relationships, social roles, deciding when to share personal information, and any other personal choices that may be relevant to the person.

“We are excited to enhance the current person-centered plan and look forward to working with each person we support as we get closer to our collective goal of providing opportunities and support for each person’s vision of a full and good life,” said Jubilee Person Centered Plan Manager Dominique Clark.
