To apply to Jubilee for yourself or a loved one, please fill out the application form on this page. A member of our admissions team will be in touch. This form is the only document required to apply for services and to be added to our wait list. Jubilee admits approximately 10 new clients each year. A more in-depth review will be part of the admissions process.

Commitment to Diversity

Jubilee is working to make disability support services more accessible to communities of color in Montgomery County. Our Breaking Barriers program aims to improve access to disability services for the underserved Hispanic and Asian communities. Jubilee’s Racial Justice Advisory Group provides guidance as we work to better serve all communities of color. 

Medicaid Funding

Most people Jubilee supports receive Medicaid funding for services through the Maryland Developmental Disabilities Administration (DDA). The process to determine eligibility and be placed on their waitlist for services with DDA typically takes up to three months. To apply for DDA funding, contact the Southern Maryland Regional Office of DDA.
